Each REPS activity can be provided once per address. If you are unsure whether you or a previous tenant may have lodged a REPS claim at your address, the YESS team can search the REPS database for you. The REPS system uses the Australia Post Address Database to determine valid addresses – if your address is not valid on the Australia Post Address Database, you are still eligible to access REPS activities; however, we will need to collect additional address evidence from you to support your REPS claim. If you have multiple business locations, and wish to order for more than one address – please fill out this form for each individual address.

Will there be sufficient space for optimal ventilaion and air circulation?
The distance from both sides and back of the RDC to the wall or other obstacle must be more than 10cm, The RDC should be in a dry place with good ventilation and located away from heat sources such as direct sunshine and stoves.
Do you have one power outlet for each RDC you are getting? The product manual does not allow installation into power boards.
For installation on the day the customer must ensure availability of 220-240V single phase 10A rated power socket outlet. The power must be reliably earthed. It is not advisable to share power outlet with other appliances; an overloaded power socket, or a power board with multiple power sockets may overheat and cause fire.
The refrigerated display cabinets you are ordering will be installed on the day (it's a condition for the REPS subsidy). This includes full unpacking and removal of all packaging materials from the site, plugging-in the units into power points provided by the customer, at the designated location inside the premises, and turning the Refrigerated Display Cabinet 'ON'. Records are made and photos are taken to confirm the installation. A user manual will be provided at the time of installation, for appropriate positioning of the unit, maintenance, and custom-setting the temperatures by the customer. Failure to adhere to the user manual instruction may void the product warranty.
This business confirms it has not had RDCs supplied to them through the REPS program at this address. If it is found upon submission of this REPS activity that RDCs have be claimed at this business address at any point under the program, the RDC units provided by YESS must be paid for at full retail price or returned in new condition.
CUSTOMER DECLARATION AND CONSUMER PROTECTION INFORMATION(Required)
•You are required to provide a digital signature with your application, which is to acknowledge the terms and conditions outlined in the REPS Information Statement, and to acknowledge the REPS Activity Record – both are part of the rebate application to REPS obliged energy retailer.
• The customer has the right to opt out of receiving the REPS energy productivity activity if they disagree with the REPS co-contribution charges, or the terms and conditions for installation, prior to the goods being brought to the premises.
• The customer must be present on the day of installation to discuss the installation or nominate a second signatory on the REPS
Information Statement who is authorised to accept/reject the service on their behalf.
• The installation shall be signed-off by the customer, or their nominated second signatory on the day of installation. The signature deems that a variation to the original REPS Order and the total cost (if applicable) has been accepted, and the customer agrees with the costs and the fees. All goods remain the property of the Installation Partner/Service Provider, until the REPS subsidy application is successfully lodged.
• The quoted amount is inclusive of REPS discounts which apply only when the REPS eligibility criteria are met. If it is found that a customer has provided false or misleading information in relation to their REPS eligibility, the customer will be responsible for the full RRP for the goods, as indicated in the REPS Order form made on the day of delivery, and a receipt provided, unless agreed otherwise in writing.
• You are not entitled to a repair, replacement or refund under the ACCC consumer guarantees if:
o they got what they asked for but simply changed their mind, found the product cheaper somewhere else, or decided they didn’t like the purchase or had no use for it.
o the problem with the product was caused by the consumer misusing it
o they knew of or were made aware of the problem with the product before they bought it (but they may be entitled to a repair, replacement or refund for a different problem with the product that wasn’t made clear to them before they bought it)
o asked for a service to be done in a certain way against the advice of the business
o the problem with a service was caused by the actions of someone other than the business.


For the purpose of the Retailer Energy Productivity Scheme (REPS), under the Electricity Act 1996 and Gas Act 1997, I hereby declare that:
• I understand that all documents regarding this REPS activity will be issued to me via the above nominated email address.
• I understand that REPS subsidy is applied on the premise that all REPS Activity-specific requirements are met.
• I understand that REPS subsidy for the premises can only be claimed once per activity, unless explicitly approved by YESS Pty Ltd, as a second visit REPS application.
• I have not previously received this REPS activity at this address, and the information I have provided is true and accurate.
• I have read and understood the above terms and conditions of the REPS Co-Contribution.
• I understand that the energy productivity activity described above is intended to be counted in satisfaction of an obliged retailer’s
obligation under the Retailer Energy Productivity Scheme.
• I understand that YESS Pty Ltd, ESCOSA, or an obliged retailer may contact me for the purpose of conducting quality assurance audits.
• I understand that YESS Pty Ltd, acting on behalf of an obliged retailer, has gathered this information for the purpose of complying with
the Electricity Act 1996 and Gas Act 1997, and I authorize it to use and disclose information about me, in accordance with the Privacy Act
1988, to its related companies and other organisations that assist it for this and related purposes.
• I acknowledge that I can access the YESS Complaints Policy and Procedure at www.yess.net.au/complaints
• I understand that my complaints or enquiries regarding the services or products provided to me by YESS Pty Ltd in relation to the Retailer
